New Client Intake

Welcome to Canines on Track!

Thank you for your interest in working with Canines on Track. Before we begin training or schedule your first service, all new clients are required to complete our New Client Intake Form.

This form allows us to gather important information about you and your dog so we can better understand your goals, your dog’s history, and any behavioural or training challenges you may be experiencing.

Agreement Overview

• Your dog’s basic information (age, breed, history)
• Veterinary and health information
• Training history and current behaviours
• Your training goals and concerns
• Household environment and daily routine
• Emergency contact information

Important Next Steps

To get started, please ensure the following are also completed:

✔ Client Agreement & Waiver
✔ Up-to-date Vaccination Records

These are required prior to scheduling your first service.
