Welcome Back to Canines on Track!

We’re excited to work with you and your dog again! Before we can schedule your next service—whether it’s training, boarding, daycare, or an enrichment session—we ask all clients to complete our Booking Request Form.

 

Completing this form ensures we have all the information we need to match you with the right schedule and provide the best care for your dog.

Who Should Use This Form

  • Returning clients whose Client Agreement & Waiver is current

  • Dogs with up-to-date vaccination records on file

  • Anyone ready to request training, daycare, boarding, or other services

If your agreement has expired or your dog’s vaccinations are not current, please submit those documents first.

Step 1: Complete the Booking Request Form

Use the form to request your desired service. Please provide:

 

  • Preferred dates and times

  • Type of service (training, boarding, daycare, enrichment, etc.)

  • Any updates about your dog’s behavior, health, or routine

  • Special requests or considerations

This helps us schedule your dog efficiently and tailor services to their needs.

Step 2: We Review Your Request

Once submitted, we will review your request and:

 

  • Confirm availability

  • Follow up with any clarifying questions

  • Send a confirmation email with scheduled service details

Submitting a request does not guarantee availability, but it starts the process and helps us reserve the best time for your dog.

Step 3: Prepare for Your Service

Before your service, make sure:

 

  • Your dog’s vaccinations are up-to-date

  • Any necessary forms (Agreement/Waiver, intake updates, etc.) are complete

  • You have all supplies ready for training, boarding, or daycare

This ensures a smooth and stress-free experience for you and your dog.
